In today's busy world many use cell phones exclusively, including me. As it seems I'm always on the move, it is much easier to collect your demographic information through my web site.
Once you have decided to make your Intake Appointment with me by phone or Email, please take a moment to fill out this form. It will allow me to open your file now, and that will save us a step later. The information is confidential and I am the only one who will see it. I will send you a confirming letter as soon as your file has been opened.
If you have not already selected a time for your first appointment you can do that by clicking on the See My Schedule option on the Navagation Menu. Select an hour and a half block of time that is open, and input that time in the appropriate place on this form. Please do not enter something like "as soon as possible." It will be much faster if you select an actual time that is available.
